If you choose to email us your order, fill out this order form and save the document to include your last name in its title.  Once you've done this, send us an email with both the order form and your photo attached to it.

If you'd like to mail in your order, fill out this order form and include both it and your photos when you mail us your order.  Consider using a photo mailer in order to protect your photographs and ensure a better quality product.

Delivering Us Your Order

If you'd like to visit us in person to discuss your order, just bring in your photos and we'll take care of the form here at the store.  Alternately, if you'd like, bring in an already completed form with you.
